Clinical Pearls

Adequate fluid resuscitation is the most critical intervention in severe burns to prevent hypovolemic shock. Lactated Ringer's is the fluid of choice.

Limitations

The Parkland formula is only a starting point. Titration of fluids should be based on urine output (target 0.5 mL/kg/hr for adults, 1.0 mL/kg/hr for children). Over-resuscitation can lead to compartment syndrome and ARDS.
