Limitations

Methadone is excluded due to its highly variable, dose-dependent half-life. Buprenorphine is also excluded as its conversion is complex and non-linear. DO NOT use this tool to calculate doses when rotating a patient from one opioid to another (cross-tolerance must be accounted for).
