FormulaStage 1: 1.5-1.9× baseline OR ≥0.3 mg/dL increase. Stage 2: 2.0-2.9× baseline. Stage 3: ≥3.0× baseline OR Cr ≥4.0 mg/dL (with ≥0.3 rise) OR new dialysis.

Clinical Pearls

Standardizes AKI diagnosis and prognostic risk across all clinical settings. Higher stages correlate directly with increased hospital mortality and long-term risk of CKD.

Limitations

Does not account for the urine output criteria of KDIGO (e.g. <0.5 mL/kg/h for 6h). Assumes baseline creatinine accurately reflects steady-state premorbid renal function.
